Chronic Care Management Program

Requirements

            • You agree that River Valley Family Health will provide CCM services to you by verbal consent. 
            • You authorize the sharing of your medical information with your treating providers by electronic communication. 
            • You acknowledge that only one practitioner can furnish CCM services to you during a calendar month. 
            • You will be billed a co-pay for a portion of CCM services even though CCM services will not involve a face-to-face meeting with the provider. River Valley will mail invoices for the Chronic Care Management Program.

What is Chronic Care Management?

Chronic Care Management (CCM) is a Medicare sponsored program, supported by the Centers for Medicare & Medicaid Services, designed, and created to allow us to monitor your health and wellbeing more closely between office visits.

How Can I Qualify?

Medicare requires eligible patients to have two or more chronic conditions. Our staff will let you know if you meet the guidelines for eligibility.

What Happens After I Enroll? 

If you consent, or verbally agree, to enrollment, then you will be contacted monthly by one of our Care Coordinators. Our team consists of nurses and Medical Assistants trained to discuss your chronic conditions and treatment as directed by your River Valley provider.  

Participation in our Chronic Care Management program will help us stay up to date on your conditions and progress. CCM also assists in coordinating care with any other provider or specialist you may be seeing. Our monthly phone calls allow us to improve communication with your provider regarding changes in your health between office visits, as well as improve your health outcomes overall.

  • COMPREHENSIVE CARE PLAN
  • DIRECT ACCESS TO ASSIGNED CARE COORDINATOR
  • MEDICATION MANAGEMENT
  • MEDICAL SERVICES COORDINATION
