JEANNIE MUELLER PA-C, CDE, RD Chief Medical Officer
Jeannie is the Chief Medical Officer (CMO) for River Valley Family Health Center with over 18 years of experience in healthcare. She is a Physician Assistant (PA-C), Registered Dietitian (RD), and Certified Diabetic Educator (CDE). She holds a Masters Degree in Science from A.T. Still University in Arizona. She is a 6th generation native of Colorado and has spent the last 17 years on the Western Slope providing care to underserved populations. As a National Health Service Loan Repayment Recipient and HRSA Leadership Award Recipient who chose to stay in rural Colorado, she understands the barriers to health care and wishes to be part of the solution. Her interests include diabetes management and pediatrics.
When she is not working, she enjoys body building and spending time with her husband and two dogs, the Brown Brothers. She is proud to be part of the River Valley FQHC team and embraces the quadruple aim of better outcomes, enhanced patient experience, reduced cost and improved clinician work life.

Assistant Chief Medical Officer
Rachel received her undergraduate degree from the University of Southern Colorado in Pueblo, (now CSU-Pueblo) and her Masters in Physician Assistant Studies from Pacific University at Forest Grove, Oregon. She has been a Physician Assistant for 10 years with experience in orthopedics and primary care. She is also a Certified Diabetic Educator (CDE) and enjoys educating patients on diabetes and their health. She started with Olathe Medical Clinic which has evolved into River Valley Family Health Center as a Federally Qualified Health Center.
Rachel and her husband have three wonderful children who enjoy the great outdoors of Colorado.
Shawna was educated at the University of Colorado at Denver where she received both her undergraduate degree and her Masters in Physician Assistant Studies. After working at the North Fork clinic at Paonia, Colorado, she joined River Valley in 2015. She primarily focuses on chronic disease management and acute care but she also enjoys dermatology and minor procedures.
Shawna has practiced yoga and/or taught it for nearly 20 years. Her practice of medicine often reflects yogic ideas combined with evidence-based medicine. Shawna is a Colorado native who loves living on the Western Slope. She likes to spend her days off hiking, biking, and skiing in the San Juan Mountains with her furry babies, Bodhi and Ziggy.
Ana is a Family Nurse Practitioner who graduated from Regis College with a Bachelor of Science in Nursing, magna cum laude and with a Masters of Science in Nursing, with Family Nurse Practitioner as the specialty area, from the University of Colorado Anschutz Medical Campus. She also completed a 12-month Advanced Practice Registered Nurse (APRN) Primary Care Fellowship at Peak Vista Community Health Centers. Ana is originally from Mexico City and is bilingual. Her passion is to serve the underserved in our community.
In her spare time, she enjoys camping and hiking with her husband and 2 dogs. She also enjoys traveling, road trips and going to the beach in Mexico.
Karie came to River Valley Family Health Center after five years in Glennallen, Alaska, where she provided primary care in a rural community health center and in urgent care. She attended Wake Forest University School of Medicine in North Carolina where she received her Masters of Medical Science, Physician Assistant Studies. She especially loves working in primary care for the opportunity to build relationships with her patients and to invest in their healthcare long term. She is also certified by the Federal Carrier Safety Administration to perform DOT physicals.
Karie’s outdoor adventures include hiking, backpacking, mountain biking, camping, canoeing, fishing, cross country skiing & snowmobiling.
Melanie graduated from the State University of New York at Stony Brook with a Masters of Science Degree in Physician Assistant Studies. Before joining River Valley, she worked in New York City as a surgical PA-C. She has also worked as a Family Medicine PA in Connecticut. Melanie is certified by the Federal Motor Carrier Safety Administration to perform DOT physicals. Melanie is a compassionate provider who thoroughly diagnoses and follows every possible avenue to lead her patients to better health. She is dedicated to serving our community.
In her spare time, Melanie enjoys spending time with her husband and 2 dogs in the great Colorado outdoors, training & running in marathons.
Joshua attended Union College in Lincoln, Nebraska, where he obtained both his bachelors and masters degrees. His Bachelor of Science in International Rescue and Relief was followed in 2012 with his Master of Science in the Physician Assistant Program. Before joining River Valley, he worked on the Navajo Reservation at Tsehootsooi Medical Center in Fort Defiance, Arizona as well as an urgent care clinic in Page, Arizona. There he gained experience in primary, pediatric and emergency room care. Joshua is licensed through the Colorado Medical Board as well as certified through the Federal Motor Carrier Safety Administration to perform DOT physicals. His passion is providing thorough, compassionate care to patients from all walks of life.
Joshua is originally from Pennsylvania but loves living in the west. He enjoys spending time with his family in the outdoors camping, hiking, climbing and skiing. He has a passion for aviation maintaining his pilot’s license and flying whenever possible. He also enjoys woodworking and loves working on projects.
Dr. Rosalind Rafanelli received her Doctor of Medicine degree at the University of Florida in Gainesville. With over 25 years’ experience in family practice in Delta and Grand Junction, Dr. Roz, brings her experience and expertise to River Valley as a primary care provider. Her medical interests are adult preventative care, wellness and healthy aging.
She grew up in Florida and while she still sails, she enjoys spending time in the Colorado mountains with her husband and two grown daughters where she hikes, fly fishes and gardens.
On Site Medical Specialty Care
Dr. Rick Reinmund attended the University of Texas at Austin for his undergraduate degree before going to the University of Texas Southwestern Medical School in Dallas. He completed his Ob-Gyn residence at Parkland Hospital, Dallas, and was in private practice in Texas for 28 years. While in Dallas, he was a Clinic Assistant Professor of the Department of Obstetrics and Gynecology at Southwestern Medical School. At RVFHC, he provides many services to women that would otherwise prove to be very costly. His medical interests include all things gynecologic: well women care, menopausal issues, contraception, sonographic imaging, abnormal Pap tests and osteoporosis. He strives to provide the best care for the women at RVFHC.
Rick is a Colorado native, having grown up in Lakewood. He returned to Colorado in 2014 where he and his wife enjoy many outdoor activities in the mountains outside of Gunnison.
A graduate of the University of Iowa College of Dentistry, Dr. Dawson has practiced for over 30 years in rural Missouri and Littleton, Colorado. Additionally, Dr. Dawson has assisted with military dental readiness for Army and National Guard troops. Most recently, he was the Clinic Dentist and Program Director for Colorado Northwestern Community College at Rangely & previously was a clinical instructor at the University of Colorado Dental School. He presently serves on the Continuing Education Committee for the Colorado Dental Association.
Registered Dental Hygienist
Elizabeth Reed graduated from Rangely’s Colorado Northwestern’s dental hygiene program in 2004. She has thirteen years of experience in periodontics, general and cosmetic dentistry and owned her own dental hygiene practice. She most recently worked for Crested Oak Dentistry in both their Grand Junction and Delta offices. She specializes in returning patients to optimal periodontal health in a non-judgmental, gentle and caring manner.
Elizabeth grew up in Montana, but has called Delta County home for the last 23 years. She is a life-long and enthusiastic Broncos fan. Between Broncos games, she is an avid landscape photographer and spends most of her free time in the mountains of Colorado and deserts of southern Utah with her husband and three dogs.

MSW, M.A., Licensed Social Worker (LSW) Therapist
A California native and UCLA graduate, Ari spent nearly 20 years living and working in Germany and earned her M.A. at Berlin’s Humboldt University in 2002. She obtained her Masters of Social Work from the University of Denver in 2011 and became a Colorado state-licensed social worker in 2013. Her medical social work and counseling experience includes post-Holocaust trauma work, palliative care, hospice and bereavement, supporting elders faced with Alzheimer’s and dementia, and intercultural work with diverse individuals and families.
A former Women’s World Frisbee Champion, Ari enjoys West African dance and spending time outdoors with her husband, her college freshman son (Go Mountaineers!), and their adopted Dachshund, Penny.
DOLORES I. MONTANEZ
Licensed Clinical Social Worker (LCSW) Therapist
Dolores joins the River Valley community as an Integrated Care Therapist who received her bachelor’s degree from the University of Illinois in Chicago, Jane Addams School of Social Work. She worked for community health centers in Chicago before she received her Masters Degree from the University of Chicago, School of Social Service Administration. After moving to San Antonio, Texas, she advanced her professional career in varied settings from psychiatric hospitals & homeless shelters, to the U.S. Air Force & Homeland Security – Immigration Customs Enforcement (ICE) detention facilities. As a native of Mexico, she speaks fluent Spanish, and professionally, she promotes treatment, education and advocacy to increase patient health & well-being.
In her spare time, Dolores enjoys Zumba classes, hot yoga, cultivating rose bushes, taking long walks and eating a big bowl of popcorn with jalapenos while watch a good thriller.
MS, NCC, Licensed Professional Counselor Candidate (LPCC)
Jenina is an Integrated Care Therapist, who is partnered with The Center for Mental Health. She completed her Bachelors of Arts degree in Psychology and Masters of Science degree in Counseling from the University of Memphis. She interned in a non-profit, urban, community mental health clinic, where she developed a passion for helping children and supporting people with trauma. She worked closely with survivors of domestic violence and is proud of the population she served. She is also a National Certified Counselor (NCC) and Licensed Professional Counselor Candidate (LPCC), and is currently working on her License Professional Counselor (LPC).
When not working, she can be found lurking in coffee shops, reading a book or attempting to write the next bestselling novel.
